PAPERWORK - After using the AED
 
Following a call out where we have used the AED (Automatic External Defibrillator) we need to complete an Event Report Form (NB this link is to a PDF file, so you'll need Adobe Acrobat Reader to read it. If you haven't got Adobe Acrobat Reader, click here for the free software) These forms are kept in the front section of the red bag. The parts to complete are:
  • Date of incident
  • Organisation responsible for the defibrillator, i.e. Welsh Ambulance Trust
  • Sex & age of patient
  • Place of collapse
  • Address of collapse
  • Was collapse witnessed?
  • Was CPR started before the defibrillator arrived?
  • What was the delay between the patient collapsing and the start of CPR?
  • What was the time delay between collapse and placing the electrodes on the patient's chest?
  • Was a shock given?
  • If YES, how many shocks were given before the ambulance arrived?
  • Did the patient have signs of circulation (life signs) AFTER any shock was given?
  • Did the patient start breathing or recover consciousness before the ambulance arrived?
  • Status of patient at end of incident
  • Additional information (if required)
Return the white part of the completed form to The resuscitation Council (UK), 5th Floor, Tavistock House North, Tavistock Square, London, WC1H 9HR, post the blue copy to National Clinical Audit Office, Swansea Ambulance Station, Cae Bricks Road, Swansea, SA5 8NS and pass the green copy on to Dem.
