PAPERWORK - After any call out
 
Following any call out we need to complete a First Responder Incident Report (NB this link is to a PDF file, so you'll need Adobe Acrobat Reader to read it. If you haven't got Adobe Acrobat Reader, click here for the free software) These forms are kept in the front section of the red bag. The parts to complete are:
  • Date
  • Time you received the call from red base (AKA ambulance control). You can get this from red base later.
  • Incident address
  • Responder PINs
  • Time First Responders mobile
  • Time First Responders on scene
  • Details of patient: age & sex
  • Call category (if known)
  • Description of incident
  • Actions taken by First Responders
  • Was defibrillator deployed?
  • Successful?
  • Additional observations/recommendations
Return the pink copy of the completed form to National Clinical Audit Office, Swansea Ambulance Station, Cae Bricks Road, Swansea, SA5 8NS. Pass the white copy on to Dem.
